Clinical and pharmacological group

The combined drug has antispasmodic, hypotensive, vasodilating effects.

Dispensed with a doctor's prescription.

How much does Papazol cost in pharmacies? The average price is 15 rubles.

Flat, cylindrical, white or white-gray tablets with inclusions. Papazol is produced in cans of 10, 20, 25, 30, 40, 50 and 60 pieces and in cardboard packages, with or without cells, of 10 pieces. Papazol UBF - in cardboard packs of 10, 20, 30, 50 pieces, in packaging with or without cells.

Each tablet contains 2 active ingredients - Dibazol in the form of Bendazole hydrochloride and Papaverine hydrochloride, 0.03 g each. The auxiliary components are potato starch, calcium stearate, lactose monohydrate.

According to the instructions for Papazol, the effect of the drug is due to its constituent components:

  1. Bendazole is a myotropic antispasmodic with vasodilating and hypotensive effects. This substance also stimulates the function of the spinal cord and has a moderate immunostimulating effect.
  2. Papaverine is an antispasmodic with a hypotensive effect. This substance also reduces vascular tone and relaxes the smooth muscles of internal organs (in particular, the gastrointestinal tract, genitourinary and respiratory systems). When taken in large doses, papaverine slows down intracardiac conduction, reduces the excitability of the heart muscle, and has a sedative effect.

Taking into account the described properties of the active ingredients, Papazol is used for:

  1. Labile arterial hypertension (high blood pressure);
  2. Peripheral facial paralysis;
  3. Residual effects after polio;
  4. Moderate spasms of peripheral arteries and cerebral vessels;
  5. Spasms of smooth muscles of internal organs, incl. for gastric ulcers, duodenal ulcers, intestinal colic, pyloric spasm, cholecystitis, spastic colitis, urinary tract diseases.

There are certain conditions under which taking Papazole tablets is prohibited. Contraindications include the following conditions:

  • epilepsy;
  • bronchial obstruction;
  • liver or kidney failure;
  • age under 1 year;
  • allergy to the components of the drug.

Doctors also do not recommend it for children under twelve years of age or people with low blood pressure, as this can lead to weakness and poor health. By ignoring these facts, you can disrupt the effect of Papazol and bring yourself harm, not benefit.

Pharmacological properties

Kartiket has an antianginal effect. It contains isosorbide dinitrate (nitrate of organic origin). Dilates peripheral arteries and veins. In this case, deposition of blood in the peripheral vascular network is observed, due to which the venous return to the heart decreases, the end-diastolic pressure decreases, and the volumetric filling of the left ventricle decreases. When used internally, it dissolves in the stomach after a few minutes. The release of the active substance from the granules is observed over a long period of time. The effect comes quickly and lasts for a long time. It is quickly absorbed and begins to act within 15 minutes. The maximum concentration in the blood is observed after 15 minutes (stays for 1-2 hours). Bioavailability is 22%, 30% of the drug is bound to blood proteins. In hepatocytes it is transformed into isosorbide-2-mononitrate and isosorbide-5-mononitrate. The half-life of metabolites is 4-6 and 1.5-2 hours, respectively. Eliminated by the kidneys in the form of metabolites.

Active substance, mechanism of its action and release form

The main active component of the drug "Kardiket" is isosorbide dinitrate. Isosorbide belongs to the pharmacological group of organic nitrates. These medications are used to relieve acute attacks of angina, as well as to prevent them. The mechanism of action of isosorbide dinitrate on the body is to expand the lumen of blood vessels due to relaxation of their walls. As a result, the load on the heart is reduced, its need for oxygen is reduced and its function improves. In addition, by relaxing the vascular walls and lowering their overall resistance, isosorbide dinitrate reduces systemic blood pressure.

Enter your pressure

The drug reduces the load on the heart muscle during an attack.

"Kardiket" is produced in tablet form in dosages of 20 mg, 40 mg and 60 mg. The package contains blisters with 50 tablets and instructions for use. The drug "Kardiket" is a long-acting drug. This means that the effect of its use occurs quickly and lasts for a long time. In addition to isosorbide dinitrate, this drug also contains additional forming substances that are responsible for the safety and appearance of the drug.

Contraindications

Absolute: • acute myocardial infarction, which is accompanied by a significant decrease in blood pressure; • shock, collapse and other forms of acute cardiovascular failure; • arterial hypotension with systolic pressure figures less than 90 mm Hg; • angle-closure glaucoma with a significant increase in intraocular pressure; • heart defects – mitral and/or aortic stenoses; • traumatic brain injury; • hemorrhagic stroke; • persons under 18 years of age (since safety and effectiveness for use at this age have not been established); • hypersensitivity to other nitrates; • hypovolemia; • combination with tadalafil, sildenafil, varnenafril (and other type 5 diesterase enzyme inhibitors) – because These drugs enhance the blood pressure reduction from nitrates. Relative: • hypertrophic obstructive cardiomyopathy (risk of increased frequency of anginal attacks); • cardiac tamponade; • severe anemia; • pulmonary edema of toxic origin; • constrictive pericarditis; • hemorrhagic stroke, cerebral hemorrhage.

Release form and composition

Round white tablets with a long-lasting therapeutic effect. On one side there is a flat section with an IR indication above the score and the numbers “20, 40 or 60” below the marks, which indicate the amount of isosobride dinitrate in milligrams. The other side is convex, with the designation of the manufacturing company SCHWARZ PHARMA.

1 tablet contains:

  • Active substance: isosorbide dinitrate – 20, 40 or 60 mg;
  • Auxiliary components: lactose monohydrate, talc, magnesium stearate, potato starch, polyvinyl acetate.

The cardboard packaging contains 2 or 4 blisters with compartments for 10 tablets. Hard gelatin capsules with a long-lasting therapeutic effect are dark beige in color, filled with white and yellowish odorless granules. The package contains 2 blisters, each containing 10 tablets.

Cautions for use

Pregnant and nursing mothers are prescribed Cardiquet only if the expected positive effects for the woman outweigh the potential risks for the fetus (child). If there is a tendency to hypotension, increased intracranial pressure, severe renal and/or liver failure, increased gastrointestinal motility, hypothyroidism, malabsorption, elderly people should be prescribed with caution under constant medical supervision. Do not use to relieve angina attacks. Prescribing the drug in high doses should be avoided - in this case, tolerance to both cardite and other nitrates may develop (cross-tolerance). When prescribed to patients with pulmonary diseases (primary), transient hypoxemia may develop as a result of blood redistribution to less ventilated areas of the lungs. If the patient has ischemic disease, this can provoke transient myocardial ischemia. In case of acute cardiovascular failure and/or myocardial infarction in the acute phase, careful medical supervision of the patient taking Cardiquet is necessary. The drug should not be abruptly discontinued due to the risk of increased frequency of anginal attacks. If a patient develops an angina attack while taking Cardiquet, it is necessary to additionally administer fast-acting nitrates. Dose selection is carried out individually depending on blood pressure. Paradoxical nitrate reactions include reactions of lowering blood pressure, orthostatic hypotension, lowering blood pressure after drinking alcohol (ethanol), during physical activity, and in the hot season. In this case, it is possible to increase the manifestations of angina pectoris up to the development of myocardial infarction or sudden death. In case of severe headache while taking Cardiquet, it is necessary to prescribe menthol-containing drugs (validol) and/or reduce the dose of Carditet. With very frequent use, addiction may develop, and even an increase in dosage is required. To prevent this condition, it is necessary to monitor the regimen of taking Cardiquet: a daily drug-free interval is necessary (usually at night for 9-12 hours). This is more effective than continuous Cardiquet therapy without intervals. Treatment is canceled if, while taking Cardiquet, severe dry mouth and blurred vision are observed, which are quite severe. During the treatment of cardite, you should absolutely not drink alcohol or ethanol-containing drugs. During the period of treatment, it is not recommended to drive vehicles or engage in work that requires increased concentration and speed of psychomotor reactions.

Use during pregnancy and lactation

During pregnancy and lactation, the drug should be taken carefully under close medical supervision.

The instructions for use indicate that adult patients drink 1 tablet of Papazol 2 or 3 times a day 2 hours before meals or two hours after them.

The duration of treatment depends on the individual characteristics of the patient and the nature of the disease. As a rule, reviews of Papazole confirm this; it lasts from 2 to 4 weeks (in adults).

The dosage of the drug and the duration of treatment in children are determined by the attending physician.

During treatment with the drug, alcoholic beverages should be avoided.

During treatment with the drug, patients may develop side effects:

  1. Increased sweating;
  2. From the central nervous system – dizziness, drowsiness, lethargy, weakness;
  3. Allergic reactions - skin rash, urticaria, facial flushing;
  4. From the digestive tract - nausea, constipation, bloating, lack of appetite, sometimes vomiting, dry mouth, changes in the activity of liver transminases;
  5. From the cardiovascular system - cardiac arrhythmia, cardiac muscle conduction disturbance, decreased myocardial contractile function, tachycardia.

The use of Papazol has virtually no effect on the central nervous system, but its excessive dosage can produce a sedative effect.

An overdose of Papazol may be indicated by the following signs:

  1. Deterioration of vision.
  2. Arterial hypotension.
  3. Feeling hot and increased sweating.
  4. Dizziness and nausea.

Treatment: discontinuation of the drug, gastric lavage with activated charcoal, use of saline laxatives. Symptomatic therapy. There is no specific antidote.

The drug is prescribed with caution for decreased intestinal motility, traumatic brain injury, hypothyroidism, prostatic hypertrophy and adrenal insufficiency.

When using the drug, it is necessary to take into account interactions with other medications:

  1. Papazole weakens the effect of anticholinesterase drugs on smooth muscle (galantamine, proserine, physiostigmine).
  2. Adsorbents, astringents and enveloping drugs reduce the absorption of the drug in the gastrointestinal tract.
  3. With simultaneous use of the drug with antispasmodics, sedatives, diuretics, tricyclic antidepressants, procainamide, reserpine, quinidine, the severity of the hypotensive effect may be enhanced.

We have selected some reviews from people who have used the drug Papazol:

  1. Tamara. I like papazole. Old drug. Verified. Reduces blood pressure gently but effectively. It works on a par with the now popular Enap. I would clarify that papazole helps me better than Enap. No side complications were observed. It's just cheap, doctors don't prescribe it.
  2. Artem. Yes, papazole really gently reduces high blood pressure due to nervous stress, overwork, etc. That is, it is a one-time use and is not suitable for permanent treatment, that is, for chronic hypertension. Papazol does not cause drowsiness and does not have a diuretic effect. The price of the drug is low, so I advise everyone to have it just in case.
  3. Veronica. Hypertension occurred on Monday. crisis /sharp drop in blood pressure/, called the emergency room. The doctor gave me 3 tablets and told me to chew them and hold them under my tongue, and then swallow them. It was papazole. After some time, the pressure began to decrease and after 3 hours it dropped to 122/74, whereas it was 161/105. I felt great, only my throat was slightly sore. I bought 2 packs for future use, but in general I always drink Lozap + in the morning and just Lozap in the evening, as prescribed by the cardiologist. I'm happy with the result.

The medicine is affordable and unique in its composition. But in the pharmacy you can find analogs of Papazole that have a similar pharmacological effect:

Before using analogues, consult your doctor.

Interactions with drugs

When co-administered with other vasodilators, drugs for lowering blood pressure, blockers of slow calcium channels, neuroleptics, tricyclic antidepressants, ethanol, quinidine, adrenergic blockers, phosphodiesterase type 5 enzyme inhibitors, procainamide, drugs for the treatment of impotence, the hypotensive effect of Cardite is potentiated. In combination with dihydroergotamine, the hypotensive effect of the latter is enhanced by increasing its concentration in the blood. A decrease in the antianginal effect of Cardite may occur when used together with sympathomimetics and alpha-blockers. The most rational combination in the treatment of coronary heart disease is the combined use of Cardiquet with nifedipine, propranolol, amiodarone. Simultaneous use with acetylsalicylic acid improves blood circulation in the coronary vessels. An increase in intraocular pressure is possible when cardite is combined with atropine or other M-anticholinergics. Absorption of cardite from the stomach is reduced by enveloping and astringent drugs.

Directions for use and dosage: Cardiket

Therapy begins with the most minimal dosages. The dose should be slowly increased to the maximum effective dose. The duration of therapy is determined individually by the doctor. It is intended exclusively for long-term use, so you should not suddenly stop using Cardiquet without consulting your doctor. Cardiket is administered orally with a small amount of water, regardless of food. Do not chew. If it is necessary to split in half, place the tablet on a hard surface so that the break line is on top and press on the line with a finger. Long-acting tablets of 20 mg are prescribed 1 piece 2 times a day. If necessary, you can increase the dosage - 1 tablet 3 times a day. Extended tablets of 40 mg are used first, 1 tablet/day (or half a tablet 2 times a day). If necessary, the dose is increased to 1 tablet 2 times a day with an interval of at least 8 hours. Long-acting tablets of 60 mg are prescribed as initial therapy, 1 tablet/day. If necessary, increase to 2 tablets/day with an interval of at least 8 hours. Extended-release capsules are used at 120 mg/day.

At what pressure should you take Cardiket?

The main indications for the prescription and use of this drug, which are prescribed in the instructions, are as follows:

  • long-term therapy and preventive treatment of coronary heart disease, which manifests itself as angina attacks;
  • complex therapy of chronic heart failure;
  • treatment of pulmonary hypertension.

Instructions for use of the medicine

Instructions for use of "Kardiket" are adjusted depending on the severity of the patient's condition, as well as on the body's response to the initial dose of the medicine. The drug at a dosage of 20 mg is taken one tablet twice a day, at a dose of 40 mg and 60 mg - once a day. According to the instructions, it is better to take the medicine before meals. If the use of the medicine in such volumes is ineffective, the daily dose of the medicine may be increased. In such situations, Cardiket tablets in a dose of 40 or 60 mg are taken 2 times a day, and in a dosage of 20 mg - 2-3 times. Only the attending physician can make changes to the dosage and frequency of use of the drug, deviating from the instructions; it is not recommended to do this on your own.

Side effects

Cardiovascular system: nitrate headaches (at the beginning of therapy) - usually disappear after several days of taking the drug; hypotension, increased heart rate, orthostatic collapse, lethargy, feeling of weakness, paradoxical bradycardia, loss of consciousness due to a decrease in heart rate (syncope), paradoxical angina. Central nervous system: drowsiness, blurred vision, stiffness, ischemic disorders, slowdown in the speed of psycho-motor response. Gastrointestinal tract: vomiting, nausea, dry mouth, epigastric pain, belching. Allergic reactions: urticaria. Others: exfoliative dermatitis, development of tolerance to Cardite or other nitrate drugs, redness of the facial skin.

Pharmacological effect

Antianginal drug, organic nitrate. Peripheral vasodilator with a predominant effect on venous vessels. Stimulates the formation of nitric oxide (endothelial relaxing factor) in the vascular endothelium, causing activation of intracellular guanylate cyclase, which results in an increase in the level of cGMP, a mediator of vasodilation.

Promotes redistribution of coronary blood flow in areas with reduced blood supply. Increases exercise tolerance in patients with coronary artery disease and angina pectoris. Dilates blood vessels in the brain and dura mater, which may be accompanied by headaches. As with other nitrates, cross-tolerance develops to the drug. After discontinuation (break in treatment), sensitivity to the drug is quickly restored.

Reduces myocardial oxygen demand by reducing preload and afterload (reduces the end-diastolic volume of the left ventricle and reduces the systolic tension of its walls). Has a coronary dilating effect. Reduces blood flow to the right atrium, helps reduce pressure in the pulmonary circulation and regression of symptoms of pulmonary edema.

The onset of action after oral administration of the drug in the form of retard capsules is 15-40 minutes, maximum action is 8 hours, duration of action is 18 hours.

Overdose

In case of an overdose of cardite, a strong decrease in blood pressure, loss of consciousness, collapse, dizziness, headache, redness of the skin, visual disturbances, vomiting, nausea, diarrhea, increased heart rate, cyanosis of the skin and mucous membranes and anoxia (due to methemoglobin formation), decreased heart rate, paralysis are observed. , coma; hyper-, dyspnea; perspiration, cranial hypotension. In case of overdose symptoms, it is necessary to inject 1% methylene blue solution into a vein at the rate of 1-2 mg/kg body weight (for methemoglobinemia). Symptomatic therapy with epinephrine is ineffective.
